Community Nursing services-Through the community nursing care program, participants are trained on how to take care of their health in scenarios such as medication, wound care, hygiene, and help with showering or dressing. There is usually a registered nurse or medical practitioner appointed who will monitor the patient’s progress and will amend the treatment and medication accordingly. Here are some of the services offered under the community nursing care program:

Community Nursing Care is provided to participants who has complex individual support needs that are best met by someone who has medical training and experience to meet the participants’ complex health needs. Our NDIS community nursing care is needed when specific individual support needs include:

Very limited communication

Behaviour support needs, and/or

Medical needs / procedures such as ventilation or home enteral nutrition

We have certified and experienced medical professionals on board with us who can render community nursing care NDIS programs to all our participants. Once you sign up with us, we allot a medical support staff who will be in touch with you to discuss, monitor, and provide feedback on your medication, treatments, and progress. Besides the NDIS community nursing care services, they will offer advice on how to engage in basic self-care and nursing where in one will become capable of doing simple things like wound care or first aid on their own.



2. Support coordination- to assist participants to achieve their goals. Goals may be NDIS related, for example, to connect with providers, or related to mainstream or informal supports. Coordination of supports can assist participants to access employment or education services or to connect with family, friends and community. Consistent with Prag Consulting’s philosophy and practice, Support Coordinators work with participants and supporters to identify what is most important and the supports required to meet identified goals.

Great Ocean Stays is a mainstream accommodation service that is fully inclusive including homes with ceiling hoists. Hospital beds, and other mobility aides are also on hand. We also have a wheelchair accessible vehicle! Our purpose built range of whee...Providing integrated support for self-care, accommodation, food & negotiated activities. This may include respite which aims to support ongoing caring arrangements between a participant and their carers. We will arrange support staff upon request (homes are otherwise not staffed). Support can be booked for short periods to 27 hour support.



Short-term respite options offer safe, secure and purpose-built accommodation for people with disabilities. It provides a beautiful setting to have time to yourself. Homes are offered for 1 participant and family/carers only (no shared NDIS participant homes). You can bring your own carers or connect with our team on site. Respite is offered in Geelong, Ocean Grove, Queenscliff & Peterborough.

SUPPORT COORDINATION

Our experienced Support Coordinators can help you navigate the NDIS system and use your plan to it's full advantage. We assist you with your plan review and guide you every step of the way. Liberty Disability Services staff have built strong networks within the agency to achieve the best outcomes for our participants. Most of all our Support Coordinators are driven to achieve positive outcomes for our participants and empower them to achieve their goals.



PLAN MANAGEMENT

As a registered plan manager we are able to give you the flexibility of having a self managed plan without the stress of having to find the finances to work flexibly within the NDIS framework. Liberty Disability Services have experienced bookkeeping and accounting staff to ensure transparency and professionalism throughout management of our your plan.



PSYCHOLOGY

Liberty Disability Services offers NDIS and privately funded Psychology services. We cover assessments, interventions'. counselling and group therapy which support both your NDIS & personal goals. Our therapists specialise in early intervention (0-6), children (6-12), teenagers (13-19), and adults (20+). Our friendly and innovative team focus on your needs and wants whilst using the latest evidence based therapies to support you and your families to build the skills needed for everyday life.



COMMUNITY MENTOR PROGRAM

Our Community Mentor Program has been designed specifically for kids and adults. We have structured the program to provide positive role models to work on skills provided by therapist and care teams in social or community settings. The program is designed to provide a "Big Brother/Big Sister" type figure for participants who are experiencing behaviours of concern, social isolation or come from complex environments. Through one on one sessions, participants will develop interpersonal social skills and build a respectful relationship with a professional, positive and fun role model.



​ACCOMMODATION & TENANCY ASSISTANCE

Finding housing can be tough, filling out all the forms even tougher! At Liberty Disability Services our staff are able to assist you navigate the wave of forms and work with you to secure accommodation and understand your tenancy obligations. Through working with Department of Housing, real estates, shared accommodation facilities or putting our heads together, Liberty Disability Services staff will work tirelessly to support you in finding a place to call home.



RESPITE SERVICES

Liberty Disability Services Respite is provided by our qualified and compassionate carers in their homes to give you the feeling of “just visiting family”. All our respite carers are individually matched to participants to ensure the right level of care is provided based on their needs. Our carers all hold current police checks, working with children registrations, home safety checklists and first aid certificates so you or the person you care for can rest assured the highest level of care will be provided.



GARDEN & HANDY-MAN SERVICES

We offer variety of house, garden and lawn maintenance to our participants. This program provides a range of services that ensures your home is kept in a suitable condition. We assist in quality up-keep in a range of tasks including mowing, landscaping, patching holes, fixing fences and doors, replacing fly screens, tree trimming, pruning, and much more.
