Our community nursing services support continuity of healthcare outside hospitalWe help mature age adults, of 40 years and over, living with a disability, to meet their goals and live fulfilled lives. We support a range of conditions including, degenerative, psychosocial, cognitive, or acquired disability. This includes stroke, early onset dementia, brain, and spinal injury as well as physical and other disabling conditions.

Our team comprises registered nurses, occupational therapists, and support workers with many years of work experiences in varied healthcare settings, including hospital, disability & community service, correctional health, aged care, and in-home care.

We provide support to manage health, engage in activities of daily living, build connections, engage in occupations, and take-up appropriate new challenges of interest, that enhance quality of life.

We assist participants by simplifying their goals, breakdown actions to easily achievable steps, prepare easy plans, up-skill and re-skill, restore relationships, community engagement/access, facilitate courses, facilitate new experiences, provide emotional support, and break barriers to achieving goals.
