CareChoice Homes is a housing service model that offers ‘forever homes’ to NDIS participants with eligible Specialist Disability Accommodation (SDA) funding. We provide our day-to-day support services in new homes from our collaborating SDA partners. Our multidisciplinary team of qualified and experienced support staff provide quality, consistent and 24-hr support.

Our homes are designed to High Physical Support, Improved Liveability, and Robust SDA design standards. CareChoice Homes offer single occupancy and shared living arrangements available throughout metropolitan Melbourne, and regional Victoria.

CareChoice acknowledges the traditional owners of the land on which we live and work. We pay our respects to all Aboriginal and Torres Strait Islander peoples and their elders past, present and future. We welcome them all to our service.

Growing up with my mum working in the industry, I’ve had a lifetime of exposure to the world of disability support. Based in Wollert, I’ve worked with individuals with a range of disabilities, including ADHD, ASD, Acquired Brain Injuries (ABI), Cerebral Palsy, and behaviors of concern.

Over the years, I’ve developed strong personal and communication skills that allow me to connect with clients and their families, ensuring they feel heard and supported. I’m dedicated to providing tailored care that empowers individuals to live as independently as possible while ensuring they’re comfortable and safe.

I believe in the importance of focusing on the strengths and interests of each individual I support. I take the time to understand what motivates my clients, tailoring my care to suit their unique skills, needs, and passions. By working towards their strengths, I aim to bring out the best in them, helping them achieve their personal goals and build their confidence.

Physiotherapy involves hands on therapy (massage), exercise and strengthening.We are physiotherapy clinic in Mulgrave, Victoria, 3170, offering musculoskeletal physiotherapy to NDIS participants and My Aged Care participants. We have immediate availability for both at home visits and in room consultations. Our particular areas of speciality are with musculoskeletal pain such as low back pain, neck pain and shoulder pain. We have worked across Victoria over the last 3 years with adult and young adolescent populations.

We provide mindful, educative and modern physiotherapy to help treat your concerns. This includes hands on therapy such as massage, home exercises for strengthening and mobility and functional daily tasks to help improve overall wellbeing.
