Hearts to Homes provide home care and assistance with daily personal activities including assistance with, or supervision of, personal tasks of daily life personal tasks of daily life. This includes personal hygiene, eating and drinking, use of aids and appliances, mobility and transferring etc. We also help with home and garden maintenance by assisting with find the right services needed. We assist with doctors, psychologist, and other specialist appointments and taking the client wherever they need to be on time.

H2H provide workplace assistance that enables an individual to successfully obtain and/or retain employment. Support can be applied to any working age individual (including students reaching working age) with an employment goal. Essential skilled staff can provide capacity building supports that are likely to lead to successful engagement in a Disability Employment Service (DES).

Here at Hearts to Homes Australia, we offer effective community challenges and participation programs for all participants. Community participation is important because it helps participants build and maintain relationships and encourages them to build their independence. These programs include:

• Recreation and leisure activities.

We believe that people of all abilities deserve access to nutrition care that considers not only their physical health, but also their emotional, social and mental health. With an emphasis on skill-building and nutrition awareness, our Dietitians aim to help their clients work towards their wellbeing goals. We provide nutrition guidance for general healthy eating, specific medical conditions, intellectual and physical disability, as well as speciality support for those who might present with disordered eating patterns. General nutrition areas may include: high cholesterol, diabetes, gut health, food allergies, low body weight, constipation and mental health. The services we have on offer include: nutrition education, grocery shopping tours, healthy cooking, flexible meal plans, menu and recipe development, staff training, nutrition supplement guidance, texture modification support and and enteral nutrition support (tube feeding, PEG). Services are available at Bondi Junction, Norwest, Gordon, in the comfort of your own home, or online via video communication. If you are a National Disability Insurance Scheme (NDIS) participant your Dietitian service may be funded through the NDIS. Depending on your NDIS plan, services may be funded under Improved Daily Living, Health and Wellbeing or Core Supports (for specialty enteral feeding resources).
