We support you as you work towards your goals, providing support coordination (level 1 and 2), support work and psychosocial recovery coaching.We come to you, assessing you in your own home, and customise what you want.Our Community Care supports our clients into the community by supporting them to:

Attending GP or specialist appointments, X-rays or hospital treatments (outpatient & inpatient) pathology, OT, Physio, Podiatry and all allied health appointments. We can assist you to be further independent by supporting you to go shopping, do errands, billpaying, re-connecting with family and friends or some extra support at a special event. Talk to us about how you access the community or how you would like to access the community and we will tailor a plan just for you.
