About Cultural Change

Specialisation - Indigenous, First NationsThe mission of Cultural Change is to empower First Nation Australians, the Elderly and CALD and People with Disabilities, to recognise and take charge of their own destiny and future through meaningful relationships, have valued roles, and be accepted for their uniqueness and cultural backgrounds.

We are able to establish an environment that develops participants' resilience and confidence, allowing them to attain their maximum level of independence.   At Cultural Change, we believe that every step we take respects the social model of disability and the biopsychosocial model of health and wellbeing, the participant's individuality and choice and control to build the participant's self-determination.

visiting friends and family attending appointments - medical, allied health active hobbies, such as painting, sewing, arts & crafts, bike riding, dancing playing sports (tennis, swimming, golf, laser tag, bowling) going out, for instance to the movies or a concert getting a massage or facial going on a holiday or visiting places for fun relaxing, like meditation or yoga learning new skills

SUPPORT COORDINATION & CASE MANAGEMENT:

case management support with home modifications, equipment purchases, allied health referrals (Self-managed HCP & NDIS) identify the services and service providers that best meet your individual needs and allow you to achieve value for money and exercise your choice and control connect and coordinate service providers and supports (including mainstream, informal, community and funded supports); or build your capacity to coordinate your own supports obtain quotes for services and set up Service Agreements with providers monitor your progress toward your NDIS goals and the performance of the support services you use review your goals prior to your NDIS plan review See more...
