Community Nursing Services We have specialist nurses who can administer any medication Conduct Assessment, Including Continence assessment and manage high support patient that require Peg feeding etc. Medication management IV Infusion Nurses deliver care personalised to your unique needs We collaborate with you / family on a care plan addressing health concerns / goals, for highest care-standard Urinary Catheterisation – (male / female suprapubic), Peg Feeds, Nasogastric Feeds Tracheostomy Care, all types of Injections, Continence Management Complex Wound Management, Ventilator Management, Complex Bowel Care

Community Participation & Access

Personal care, meal preparation Housekeeping, shopping, and transport. We develop a personalised care plan to suit requirements. Organise activities: daily exercise classes to cinema outings. Assistance with travel and transport Development of daily living and life skills Work with you on personalised plans for needs / interests. Helps you develop new skills, build relationships, and foster belonging in your community. Helping people connect with social options that meet their needs and interests

SUPPORTED INDEPENDENT LIVING (SIL)

Participants can access core supports in the home to maximise their quality of life. This works great for our clients that need daily support services. Provides our clients with an opportunity to meet new friends in our SIL accommodation or in our community. Through our SIL services at True Living, our clients are able to open themselves up to enjoy new experiences in the community and within our SIL homes.
