Helping individuals with disabilities access and navigate the services they needAt Shukran we work with participants to obtain medium to long term housing with appropriate supports to maintain their tenancy, engage in community through accessing appropriately linked community care services.

We provide participants with skilled, practical and positive support aimed at improving quality of life as well as enriching life experience through family and community connections. This is achieved through a client led and person-centered approach.

Our support staff will work with participant’s families and carers to reach their goals. We listen to and, in partnership, develop personalised plans and supports that ensure supports are reached as desired.
