People with a disability and our elderly are often talked about as if they are in one group, but we know that every individual participant at Horizon Community Services has different needs, we endeavor to ensure that we provide a service that meets those needs.

Providing care that has been completely personalized for every individual is important to us here at Horizon Community Services. We recognize every individual as the unique person that they are, and it’s why we avoid a solution that is “one thing fits all”. Our care plans are person-centered, which means that you get a service that shines a light on what you need and delivers a solution for it.

Horizon Community Services is aware and appreciative of the passion for those with disabilities and elderly to remain as independent as possible in their home. It’s for this reason that we ensure that we provide a range of services that are tailored to allow this to happen. Our disability support services include: Domestic assistance, Respite Care, Personal Care Community Access, Nursing Support and Professional care support.

We have extensive experience in disability and elderly care and specialize in tailoring all levels of supports to ensure our participants reach their goals.
