• 0101 - Accommodation/Tenancy • 0104 - Assist Personal Activities High • 0106 -Equal Access Care is a dedicated NDIS provider committed to delivering comprehensive support services tailored to individual needs. We offer assistance with accommodation and tenancy, personal activities (both regular and high-level), and life stage transitions. We also provide essential services such as community nursing care, daily tasks in shared living environments, and innovative community participation programs. Additionally, Equal Access Care helps with travel and transport, development of life skills, and managing household tasks, ensuring participants have the support they need to lead fulfilling and independent lives.

We are currently taking referrals for Level 1 and 2 Support Coordination within the Melbourne area.All levels of Support Coordination are available, including:

Level 1: Support Connection.

Level 2: Coordination of Supports.

Level 3: Specialist Support Coordination.



All age groups and levels of ability are supported, including:

NDIS plans for YPIRAC (Younger People in Residential Aged Care).

Participants living with psychosocial disabilities.

Children and Young People living in Statutory Out Of Home Care (SOOHC) or Voluntary Out of Home Care (VOOHC).

Acquired Disabilities.



As a participant with Trusted Support Coordination, you will have an assigned Support Coordinator to work with you and your NDIS plan. You will have the opportunity to strengthen your skills and knowledge to best meet your NDIS plan goals. The Support Coordinator will work closely with you to ensure you get the best out of your funded supports, informal supports and mainstream supports.

Cultural Sensitivity: Trauma informed approach and specialised care and support for Aboriginal, Torres Strait Islander, CALD, and LGBTIQA+ communities.

We understand the unique challenges faced by Aboriginal and Torres Strait Islander peoples, by those from CALD backgrounds and those in the LGBTIQA+ community. We recognise the struggles and draw parallels between marginalised communities and people living with disability in order to better understand and to offer culturally sensitive care and support. We actively recruit our workforce from these communities to better represent the people that we provide care and support to.

Unique to On Your Marks Care is our lived-experience workforce (peer support workforce) embedded at every level of the organisation. This ensures that a participant lens or focus is adopted at every meeting, planning session, and service review. Our commitment to mutuality, supportive and empathic approach is demonstrated through this workforce. We have peer support workers from mental health, forensic and addiction background so we can offer another level of support should the participant want.

We believe that a trauma informed care approach should underpin everything we do and so ALL of our workforce (regardless of whether they are participant facing role or not) are trauma informed.

We use exercise and personal training in combination with other supports in order to assist those with mental health and addiction needs. We believe that physical activity, sport, exercise and regular physical training improves overall health, reduces stress and anxiety that is often associated with living with a disability, can regulate mood and help stabilize mental health.
